I would not say that you have too but you would see a performance gain as well as MPG increase. The screw is below the one that is the handle. The one with the handle is for idle adjustment. No you can do this with the prop on. You won't have to go up to full throttle about 2500-3000 rpms. Look at the second pic in the link I posted B & C are thre timing advance screw.
